Food for Thought: Weekly Wrap-Up

Does Germany need more than 500 cancer centers? After attending a meeting of representatives from 10 university cancer centers and the National Center for Tumor Diseases (NCT), Rainer Flöhl in Frankfurter Allgemeine Zeitung (FAZ) is skeptical. While the university cancer centers are similar to the successful Comprehensive Cancer Centers in the US – linking basic and clinical research (which is called “translational research” nowadays), they do not yet provide nationwide cancer care. Therefore, the German Cancer Society supported the adding of further centers: “clinical oncology centers” and “organ centers”. At present, Flöhl writes, the landscape is dominated by organ centers (e.g., 200 each for breast and colon cancer), however they lack a multidisciplinary approach and are hampered by poor documentation standards and poor financing. As a result, patients do not get optimum treatment and many cancer centers may have to close sooner or later.

In Die Zeit, Gianna-Carina Grün is dealing with the aftermaths of the German EHEC epidemic. Not only modern sequencing technology but also a collection of strains played a crucial role in the rapid identification of the EHEC strain. However, Europe does not have comprehensive collections of microbial strains – building such databases is tedious and does not provide for scientific glory in terms of publications and inventions. Grün explains that not lack of funding, but structural deficits in academic and public institutions have prohibited the building of valuable databases so far.

Gert Antes, director of the German Cochrane Center in Freiburg, in Süddeutsche Zeitung (SZ) introduces the Cochrane library which meanwhile lists 600,000 randomized, controlled clinical studies from all over the world. He also features efforts by the US, Canada and various European countries to provide patients and health care professionals with reliable information distilled from the library. Germany, Antes states, does not invest in any attempts to turn research results into practice and to find what really works in health care. He is also disappointed by the German government´s plans to cut funding of clinical research. Antes’ conclusion: “Germany has never had a role as trailblazer in patient-oriented research, and now it seems it is heralding the end of it.”

Ulrike Gebhardt in Neue Zürcher Zeitung (NZZ) provides an overview on how immune cells cooperate with cancer cells. As an example, she mentions M2 macrophages which are tricked by a tumor so that they mistake it for a wound. As a result, they start a support program to direct blood vessels and nutrients into the area. In addition, they support cancer cells in forming metastases. The findings, Gebhardt states, may lead to novel ways to cure cancer and to prevent metastasis.

Farhad Manjoo in The New York Times (NYT) introduces the latest health-related Apps and gadgets to monitor activities and body functions such as heart rate, blood pressure and temperature. Joshua Brustein, also in NYT, introduces the latest internet startup prone to change our way of life: Taskrabbit, a market where busy people can hire others to help them with everyday tasks such as picking up groceries, making reservations and assembling IKEA furniture.

Finally, for people who always wanted to know whether they are descendants of Tut Ankh Amon, Swiss iGenea offers an answer. Turn in a swab of your DNA, and if your genes are the closest match to the pharao’s DNA, you get your money back. iGenea’s regular business is genealogical DNA analysis, and the company promises to provide its customers with insights about origins and migrations of their ancestors and information on whether they stem from ancient tribes suchs as vikings, celts or jews. However, as Matthias Glaubrecht explains at length in Die Welt, it is far from being clear that iGenea has obtained the mummy’s full DNA profile, let alone that the DNA derived from the mummy is indeed DNA from the pharaoh and not a contamination. In addition, as Eva Zimmerhof writes in Focus, preliminary tests demonstrate that about 45% of German, 50% off Swiss, and up to 80% of Spanish males are descendants of Tut Ankh Amon.

Andreas Sentker in Die ZEIT takes up the issue of rising eco-terrorism in Germany directed against field trials of genetically modified plants. These plants are an easy target as trial sites have to be disclosed to the public in registers following legislation initiated by the former red-green coalition in Germany. While in the past years opponents of genetically modified plants only targeted the plants they dub as “Gen-Dreck”, violence is now also directed against security firms and guards watching the fields. Last week, a masked gang assaulted security guards, looted their mobile phones and destroyed different plants of research projects from the University of Rostock. The plants were carrying genes for the production of biopolymers and for a vaccine against viral diseases.

In Süddeutsche Zeitung (SZ), Hanno Charisius introduces efforts to create artificial leaves mimicking photosynthesis to create hydrogen which can be used as fuel or energy source. Daniel Nocera from the Massachusetts Institute of Technology (MIT) is just using the principle: water molecules are split using electrical current produced by solar cells – an ages-old principle. Noceras new twist, however, is the use of a mix of cobalt and phosphate as a catalyst. The mix is accumulating at the electrodes, but after a while it is crumbling and thereby regenerating. The reaction is not the most effective one, but prototypes are already running for months without efficiency drop. First machines will now be installed in India for tests in realistic  conditions; the goal is to meet the daily power demand of an Indian family by using just 4 liters of water and sunlight. Nocera is now trying to combine his catalyst with solar cells to a single device.

Katrin Blawat, also in SZ, in a somewhat pessimistic article deals with the many failures to develop a drug to combat Alzheimer’s disease. Given that there are neither cures nor effective therapies to prevent or delay the onset of AD, she cautions against tests for early diagnosis.

Another negative outlook is given by Werner Bartens in the same paper. Bartens deals with personalized medicine, calling the concept a “set phrase” invented to disguise that personalized medicine is a mere “PR strategy by the pharma industry and stakeholders from academia” invented to lay a smoke screen on the failure to develop new blockbuster drugs. For Bartens, the “niche buster concept” is “science fiction” allowing the pharma industry to obtain approval without any proof of patient benefit. As an example, Bartens interestingly mentions the introduction of personalized cancer drugs of which only a fraction of the patients is benefitting.

In contrast, Ronald D. Gerste in Neue Zürcher Zeitung (NZZ) writes about how cancer patients have profited from targeted therapies, e.g. in terms of decreasing cancer mortality and improved survival. Gerste features a trial by the MD Anderson Center in Houston/Texas in which 460 cancer patients underwent detailed molecular genetic analysis. Subsequently, they were treated by a matching therapy addressing the most promising molecular mechanism. Without applying any experimental drugs, this procedure increased survival from 9 to 13.4 months on average.

Also in NZZ, Stefan Betschon makes the case for better science communication, introducing an article by Dirk Helbing and Stefano Bialetti on “how to create an innovation accelerator“. They propose to improve the academic publication system by publishing articles ahead of peer review finalization and call for review by crowd sourcing. Rejected publications should also be made public, alongside with comments.

The Economist this week casts doubt on whether the goal to eradicate polio by the end of 2012 can be met. The disease is still endemic in four countries (Afghanistan, Pakistan, India and Nigeria), however re-emerged in African and Asian countries in recent years. Reasons are political (war, refugees, etc.), socioeconomic (poor sanitation) and medical (vaccines needs cold chain, virus can hide in asymptomatic carriers) factors.

Another article in The Economist deals with epigenetics findings by a German research group from the University of Konstanz. The team found that stress during pregnancy (abuse, violence, famine, death of a relative etc.) can lead to a change in the DNA methylation patterns of the unborn child, e.g. of the gene coding for glucocorticoid receptors which relay signals from stress hormones in the blood to cells of the brain regions controlling behavior. The findings may explain why children of stressed mothers show higher-than-normal rates of psychological and behavioral disorders and may lead to insights about how and when interventions are possible and promising.

Andrew Pollack in the New York Times features discussions on changing the rules for research in human subjects. The US government claims that changes are needed in the light of genomics studies using patients’ DNA samples, the use of the Internet and a growing reliance on studies that take place at many sites at once. Among the proposed changes are adding informed consent rules to donors of blood, DNA or tissue samples and allowing a single institutional review board for multiple-site clinical studies.

And finally, for all of us struggling daily with the touchscreen keyboards of our iPads and iPhones, IBM comes with a solution: a keyboard that morphs to fit an individual’s finger anatomy and typing style. Paul Marks in Wired dug out an IBM patent describing keyboards in which buttons are automatically resized, reshaped and repositioned according to the users typing style.

Hildegard Kaulen in Frankfurter Allgemeine Zeitung (FAZ) reports from the 61st Lindau Meeting of Nobel Laureates on the need for novel antibiotics. She features the talk of Thomas A. Steitz from Yale University on ribosomes and novel antibiotics. Steitz in 2009 received the chemistry nobel prize for the structure determination of ribosomes together with Ada Yonath and Venkatraman Ramakrishnan. This discovery has led to novel insights on antibiotics binding to these cellular organelles – an important prerequisite for the design of novel antibiotics as bacterial ribosomes still are the most important targets for antibiotics. Among others, the scientists learned that the larger the contact area of ribosomes and an antibiotic, the more mutations are necessary to evade the binding and anti-microbial activity of the compound. Steitz therefore recommends linking antibiotics. He also co-founded a company, Rib-X Pharmaceuticals, which is designing novel antibiotics by structure-based design. Its most advanced compound successfully completed a Phase II study this year.

Richard Friebe, also in FAZ, reports on a breakthrough in synthetic biology accomplished by a team of German, French and Dutch scientists and published in Angewandte Chemie. Other than Craig Venter, who rebuilt an organism by chemically synthesizing its DNA, the group designed a partially artificial organism. Using automated selection, the researchers transformed an E. coli strain unable to synthesize thymine nucleotides into an organism incorporating the artificial thymine analogue 5-chlorouracil instead of thymine into its entire DNA. The goal of the project was to demonstrate that it is possible to develop a generic technology for evolving the chemical constitution of microbial populations by using the simplest possible algorithms. Members of the team recently co-founded Heurisko USA Inc.

Die Welt reports on novel insights into the medical role of Helicobacter pylori, a bacterium living in the human stomach and known for its ability to cause gastritis, gastric ulcer and stomach cancer. Christian Taube from the University of Mainz and colleagues from Zurich University recently published findings that early infections with Helicobacter can protect against allergic asthma. In newborn mice, an early infection impaired maturation of dendritic cells in the lung and increased enrichment of regulatory T cells responsible for oppressing asthma. Resistance is lost once Helicobacter is eradicated with antibiotics. The researchers therefore think that the increase of allergic asthma may be caused by today’s widespread use of antibiotics.

Type 2 diabetes can be cured by a strict diet, reports Christina Berndt in Süddeutsche Zeitung (SZ). In a UK study comprising 11 type 2 diabetics, in 7 of the patients insulin production normalized and the liver started to respond to the hormone properly after they were put on a strict 600 kcal diet for 8 weeks. The cure even worked in patients suffering from diabetes for 4 years and the effects were lasting, provided the patients did not overeat subsequently.

William Pentland in Forbes writes that the US Defense Advanced Research Projects Agency (DARPA) is a driving force behind a new effort to harness biology as a manufacturing platform. The “Living Foundries” program is designed to fund projects that enable on-demand manufacturing capabilities for the production of advanced materials and devices. “Key to success,” DARPA writes, “will be the democratization of the biological design and manufacturing process, breaking open the field to those outside the biological sciences.” As examples, DARPA mentions next-generation DNA synthesis and assembly technologies, modular genetic parts and systems, and cell-based fabrication systems.

In a Forbes interview conducted by Alex Howard,  Charlie Quinn, director of data integration technology at the Benaroya Research Institute, talks about the necessity of new tools and strategies to cope with today’s data deluge. Quinn, who is dealing with genomics, maintains that it is not only about novel technologies but also about cultural changes to create greater value by sharing data and establishing open source and even open data projects, sharing data much earlier than it is done now. Thereby, novel ideas can be spread earlier. “What we’ve been doing is going around and trying to convince people that we understand they have to keep data private up to a certain point, but let’s try and release as much data as we can as early as we can.”

Rationing medicine already is clinical reality in Germany, reports this week’s Frankfurter Allgemeine Zeitung (FAZ). Christina Hucklenbroich features a representative survey among the members of the German Society for Hematology and Oncology (DGHO) about therapeutic decisions in treating cancer patients. According to the survey, 59% of the responding 345 oncologists said that they abstain from treatment options if they think the therapeutic benefit is too small as compared to the cost of treatment. However, 19% responded they even refrain from therapeutic options for cost reasons even if the treatments provide an additional, considerable benefit to the patients.

Michael Feld also in FAZ reports on a study by Pricewaterhouse Coopers and the Darmstadt Economics Research Institute Wifor that Germany will be lacking 56,000 physicians and 140,000 nursing staff by 2020, a situation that will hit the eldery most. The author, a practicing physician, states that the situation is not only caused by lack of money but also by disappearing values like charity, social responsibility and a sense of honor.

Focus magazine this week features a study from the University of Michigan giving rise to concerns that taking dietary supplements and OTC medications to stimulate the immune system can be counterproductive in patients with autoimmune diseases. The study demonstrates in animals that a strong immune response to common cold viruses can exacerbate inflammations and even lead to asthma attacks while the infection with a weaker immune system proceeds without complications.

Die Welt reports about clinical results on a new test for the early detection of Alzheimer’s disease (AD) communicated by the University of Leipzig. The test is based on radiolabeled Florbetaben which is injected into the blood stream. The substance binds to beta-amyloid peptides in the brain, and binding can be assessed using PET imaging. Thereby, AD can be diagnosed up to 15 years before onset of the disease. The paper does not mention, however, that the (preliminary) results are from an international multi-center Phase III trial sponsored by Bayer Schering Pharma that was designed to evaluate the efficacy and safety of florbetaben (BAY 94-9172) developed by the company. PET images are compared to corresponding histo-pathological specimens. Details will be published in the next issue of Lancet Neurology.

Christian Meier, Aitziber Romero and Dino Trescher in Sueddeutsche Zeitung (SZ) maintain that industry is trying to block attempts to regulate nanotechnology products. While the EU Commission prefers to define nanotechnology products by counting the number of particles smaller than 100 nanometers, industry wants a definition by determining the fraction of the particles contributing to the mass of the product. The authors, which claim that nanotech products bear all sorts of unforeseen health and environmental hazards, say that this is an attempt by industry to reduce the number of products defined as being nanotechnology.

The Economist makes a case in how food poisoning by EHEC, salmonella and other dangerous bacteria can be effectively prevented: radiating food. Irony is that it was Germany, the country currently suffering from the worst and most deadly EHEC epidemic ever, that vetoed a proposal by the European Commission to allow radiation for a greater range of food and at higher doses, e.g. for sprouts which caused this year’s epidemic, in 2000. However, the author doubts the epidemic will change the German government’s attitude for fear to upset Germany’s influential Green movement.

Last not least, comics are becoming increasingly popular among biotech companies and researchers. Silver Spring, MD based biotech company United Therapeutics chose to publish its annual report as a comic book, while researchers from the Department of Neurosurgery of Heinrich-Heine-University Düsseldorf published a retrospective study on traumatic brain injuries in comics, analyzing more than 700 head injuries in the Asterix comic books: “Although over half of patients had an initially severe impairment of consciousness after TBI, no permanent deficit could be found. Roman nationality, hypoglossal paresis, lost helmet, and ingestion of the magic potion were significantly correlated with severe initial impairment of consciousness (p ≤ 0.05).”
